This questionnaire was designed to help you ask questions. Making the decision to place your child for adoption is a very important decision for not only your child's well being, but also your own.

Once you have completed the form, your counselor will analyze your situation with you and help you work through any issues or questions you may have. Please remember there are no right or wrong answers, so answer as honestly as you can.
